the world border did not expend becouse the clocks are in the overworld,
when you died the chunk got loaded again and you got the achievement.

The command blocks that control the world border are in the overworld, so
having you both in the nether messed it up a bit. Hopefully it gets sorted
out.

When the game has obvious glitches and bugs you should just “cheat” some
stuff back in. You may have had time to recover the sapling without the
mysterious suffocation in the nether.
